Epic MyChart is Osler's new secure online patient portal. It gives you access to your health information from your computer, tablet or mobile device.
As an Osler patient, you can use Epic MyChart to view information from your hospital visits, stay informed about your care and share information with family members, caregivers and members of your health care team.

With Osler’s new Epic MyChart you can:
- View your laboratory and diagnostic test results, some clinical notes and after visit summaries
- Access patient education materials
- Manage appointments as well as receive reminders and preparation instructions
- Update your medication list
- Request a download of your full medical record
- Complete consent forms and questionnaires
- Join virtual appointments
- Pay bills
- Share access with a proxy (a family member or a caregiver, including health care providers outside of Osler)
- Prepare for surgery using the MyChart Care Companion feature
MyChart Care Companion
For scheduled surgery patients, Epic MyChart offers personalized surgery preparation planning, reminders, and recovery guidance through MyChart Care Companion—an interactive platform that allows for home monitoring, designed to help patients, their loved ones and their care teams. Patients are auto-enrolled into MyChart Care Companion when they are booked for surgery.

Frequently asked questions
Why is Osler changing its MyChart platform?
Osler is implementing Epic as its new hospital information system to create one centralized digital health record for each Osler patient. The new Epic MyChart will connect patients and families to personal health information from their health record and support safer, more coordinated care across Osler.
All Osler patients 12 years and older can register for Epic MyChart.
Can parents or guardians access a child’s Epic MyChart account?
Osler patients over the age of 12 will be able to have their own Epic MyChart account, and they can provide access to their parents/guardian.
How do I create an Epic MyChart account?
Easy ways to create an Epic MyChart account:
- Online through the Epic MyChart patient portal
- Using an activation link sent by Osler
- During registration for an appointment or hospital visit
What information do I need to activate my account?
Osler patients will need their health card number to activate their new Epic MyChart account.
Will I be able to see all my previous MyChart records in the new platform?
After October 3, 2026, you will have access to five years’ worth of previous MyChart records in the new system. Patients will have access to this information until July 2027.
How do I find Care Companion in Epic MyChart? Who can I contact if I am not auto-enrolled?
Once logged in, all Care Companion content will be under the ‘To Do’ section of MyChart. This includes viewing all care plans you are enrolled in, past finished tasks, and adjustments to reminders. If you have been scheduled for surgery and are not auto-enrolled in MyChart Care Companion, please speak with your care team.
Can I access Epic MyChart in different languages?
The menus and navigation in the Epic MyChart are available in:
- English
- French
- Simplified Chinese
- Spanish
- Arabic
Clinical documents and forms are available in English only.
Is there a cost to sign up for Epic MyChart?
There is no cost to sign up for Epic MyChart.
What if a patient needs records older than five years?
Patient records at Osler are available through Osler’s Health Information Management team.
Epic is widely recognized as a global leader in hospital information systems and is a safe and easy way to see all your up-to-date Osler health information online. If you have questions about privacy, contact Osler's Privacy Office.
As with any health information system, patients should protect their login details and only share access with people they trust.
Can a family member or caregiver access my information?
Yes. Epic MyChart supports proxy access, allowing patients to share information with family members or caregivers. The Epic MyChart will allow you to grant proxy access directly in the platform. For children under 12 years of age, parents/guardians will be able to create an account on the child’s behalf and have primary access.
Proxy access allows a patient to give another person, such as a family member, caregiver or health care provider, access to their Epic MyChart information.
Will Osler’s team help with proxy access?
Yes. Osler’s Health Information Management team will continue to support proxy access if needed.
